TO APPLY FOR A LIBRARY CARD

  1. Fill out a registration card

  2. Must show I.D. with current address

  3. Must be a resident of Rutherford or Polk County, be employed in the county, or own property in the County

If you have a plastic library card from another library in the county, you must fill out a registration card and show I.D.

For the first visit, adults may choose one book, and children may choose two books.

Items checked out on first visit must be returned before additional items may be checked out.

  • This card may be used at all CMC Consortium Libraries
  • This card is non-transferable
  • Present this card each time you check out items or use the Internet
  • Report lost card immediately
  • A fee will be charged for replacement
  • Inform the library of any change of address
